Example wording

This program is dedicated to [Name], whose patient love shaped our family and whose memory will remain part of our everyday lives.

Make it fit your situation

If several relatives sign it, agree on the collective wording before printing.

For an individual dedication, write: For [Name], with gratitude for the love and example you gave me. This distinguishes a personal tribute from a statement made by the whole family.
